First trimester vaginal bleeding is a common presentation, occurring in 20 to 40 percent of pregnant women. Bleeding can be light or heavy, intermittent or constant, painless or painful. In most cases, there will be no long term sequelae, however it is important to exclude other common first trimester complications such as miscarriage or an ectopic pregnancy.
Urgent medical review is recommended whenever bleeding occurs in pregnancy. This is usually followed by an ultrasound assessment of the pregnancy to determine its location and viability. If any uncertainty remains, then serial ultrasounds or serial blood test looking at pregnancy hormone (BhCG) levels may be required.
